Service Description
During this visit the client will receive a nutrition assessment as it relates to a medical diagnosis, work with the provider to determine personal goals, and discuss recommendations. Note that this type of visit may be reimbursed by the client's health insurance. Client must be a NH resident.
Cancellation Policy
Payment for services must be made prior to meeting. Payment can be made via credit card, Venmo, cash, or check. Appointment cancellation must be made 24 hours for a full refund. Cancellations after that time will result in a charge of the $60.
